Proof of employment for eligible sectors is required and may include: pay stub, ID badge, or letter on company letterhead indicating employment. Please note: residents who book a vaccination appointment through a community or hospital vaccination clinic that utilizes the MyTurn, the State’s scheduling system, will need to check the ’65-74’ age range option to get the scheduling process started. This is due to a current limitation within the State’s online scheduling system. You will then be prompted to enter your date of birth at which time you will enter your actual date of birth (the 65-74 age option is only to get the process started, use your real date of birth after that point).
